1For children/youth under 21, contact PerformCare (877-652-7624) which provides a family-centered, community-focused, single point of entry to obtain services for behavioral health, intellectual/developmental disabilities, or substance use. Available 24/7. Parent/legal guardian consent required for youth unter 18.
2Immediate rescue required for self-injury, serious threats of harm to self/others, severe disorientation, and/or agitation, etc.
3Crisis stabilization provides short-term care for an individual experiencing an acute mental health or substance use crisis.
